Introduction: The bar attachment is a robust cast metal retention device that connects roots, crowns or dental implants.In the case of mandibular subtotal edentulism, balance problems arise due to bone resorption and restricted bearing surfaces.This study aimed to describe the benefits of the bar attachment in the management of mandibular subtotal edentulism.
Observation: Two patients were referred for functional and aesthetic prosthetic rehabilitation. Clinical examinations revealed maxillary partial edentulism and mandibular subtotal edentulism. Radiographic examinations of the mandibular canines confirmed a radiological crown/root ratio of 1. Articulator cast examinations showed sufficient prosthetic space in both cases. For the first
patient, a maxillary temporary removable partial denture and a mandibular bar-retained overdenture were indicated. The second patient received a maxillary temporary removable partial denture and a fixed removable prosthesis retained by an anchoring bar connecting two fixed crowns. Particularities were observed during dental preparations, impressions taking and prostheses fabrication in the dental laboratory. On the day of fitting, instructions for use and maintenance were provided to the patients.
Conclusion: The use of the bar attachment as a retention system in the management of mandibular subtotal edentulism offers an effective alternative to conventional full dentures, provided that a rigorous treatment plan is established and postprosthetic maintenance is ensured.
