| RÉSUMÉ
Introduction. Définie comme une solution de continuité sur la chaine musculo-osseux-tendineux du genou assurant l’extension de la jambe sur la cuisse, la rupture traumatique de l’appareil extenseur du genou est courante dans notre exercice. Elle est dominée par les fractures de la rotule. L’atteinte du tendon rotulien ou quadricipital est peu fréquente voire exceptionnelle
dans une topographie bilatérale. L’objectif de ce travail était de rapporter un cas clinique de rupture bilatérale du tendon rotulien de diagnostic tardif, qui est survenue chez un adulte avec une insuffisance rénale chronique en traitement depuis 5 ans.
Observation. Il s’agissait d’un adulte, qui avait présenté une rupture bilatérale de l’appareil extenseur du genou suite à un traumatisme minime. L’anamnèse avait permis de retrouver un antécédant insuffisant rénal chronique correctement suivi. Le diagnostic de rupture bilatéral de l’appareil extenseur du genou été tardif ainsi que la prise en charge chirurgicale. La
prise en charge avait été réalisée grâce à la technique de Krackow suivie d’une protection par un cadre métallique. Après 8 mois de suivi post opératoire le résultat fonctionnel avait été acceptable.
Conclusion. La rupture bilatérale du tendon rotulien reste une entité pathologique rare, qui semble toujours associée à un terrain particulier. Le retard de diagnostic (et donc de prise en charge) est un important facteur de pronostic fonctionnel.
ABSTRACT
Introduction. Defined as a solution of continuity on the musculo-osseous-tendon muscle of the knee system ensuring the extension of the leg on the thigh, the traumatic rupture of the extensor apparatus of the knee is common in our exercise. It is dominated by the patella
fractures . Patellar or quadricipital tendon involvement is rare or even exceptional in a bilateral topography. The aim of this work was to report a clinical case of bilateral
rupture of the patellar tendon of late diagnosis, which occurred in an adult with chronic renal insufficiency under treatment for 5 years.
Case report. It was an adult, who had a bilateral rupture of the knee extender due to minimal trauma. Anamnesis found a chronic kidney disease correctly follow-up. The diagnosis of bilateral rupture of the device extends of the knee was late as well as surgical management. Surgical management had been carried out thanks to Krackow’s technique followed by a metal
frame protection . After 8 months of postoperative follow up, the functional outcome was acceptable.
Conclusion. The bilateral rupture of the patellar tendon remains a rare pathological entity, which always seems to be associated with a particular terrain. Delayed diagnosis (and therefore management) is an important factor in the prognosis of function. |
